New patient consultations for children are a courtesy. There is a fee for second opinions when a patient is already in treatment with another provider and for all adults 18 and over.

We ask that you complete and submit the online health history form before your appointment and that you arrive on time. Please plan to spend an hour and a half so that we may have time for a thorough examination and explanation of any proposed treatment.

From the 101 Freeeway:
1. Take the Westlake Boulevard exit & turn north (right if you're coming from "The Valley", left if you're coming from Ventura). The Promenade will be on your right.
2. Cross over Thousand Oaks Boulevard and at the next traffic light, turn left into North Ranch Mall.
3. Make another quick left into the parking lot. We are on the 2nd floor above the Santa Barbara Bank & Trust.
4. The building's main entrance faces Westlake Boulevard.

From Burbank Airport:
1. Start out going SOUTH on N HOLLYWOOD WAY toward W PACIFIC AVE. (go 3.0 miles)
2. Turn RIGHT onto W ALAMEDA AVE. (go 0.0 miles)
3. Merge onto CA-134 W. (go 1.7 miles)
4. Merge onto US-101 N. (go 27.1 miles)
5. Take the WESTLAKE BLVD exit toward CA-23 S. (go 0.4 miles)
